Licensed Vocational Nurse (ICP)

UMMA Health, the first Muslim American founded community-based health organization in the United States, is hiring a Licensed Vocational Nurse for its Integrated Care Program (ICP) clinic at 6814 Pacific Boulevard, Huntington Park, CA 90255.

Position Overview

This full-time role divides equally between direct clinical patient care coordination and community outreach activities, serving enrolled program participants in an integrated healthcare setting that addresses both medical and social health needs.

Key Responsibilities

Clinical Coordination (50%)

• Evaluate and monitor patient treatment plans and ongoing well-being
• Create and update comprehensive case progress reports
• Deliver health education focused on chronic disease management and prevention
• Participate in weekly multidisciplinary case conferences with clinical updates
• Manage clinical supply inventory
• Perform basic clinical procedures within LVN scope of practice
• Collect and enter patient outcome data into the electronic health record (EHR) system

Community Engagement (50%)

• Coordinate and facilitate community outreach events and activities
• Engage patients and families in health promotion initiatives
• Support community health activities aligned with program goals

Required Qualifications

• Graduation from an accredited LVN nursing program
• Active California LVN license or current interim permit
• Current CPR/BLS certification
• Practical experience in public health and community clinic nursing settings
• Strong interpersonal and organizational skills
• Ability to work compassionately with patients experiencing mental illness or physical disabilities
